I finally switched out my TI-101 for a Telos ONE (rackmount).
Here's my issue:
The input is bleeding back out of the output.
In other words, I can hear a "phone ghost" of myself with a short delay.
I've set the gain on the front panel correctly and the caller hears everything just fine.. but I hear myself with a short delay.
I understand if I were using the AUX xlr out which has the option of sending the mix or aux, but I'm using the OUT (the third from the left or far right xlr on the rear).
Do you think the phone wire quality has something to do with it? I just bought a quick 99cent-store phone wire to test it... maybe there's bleed-through on there do to a lack of insulation?
I'm waiting on a return call from Telos, but thought I'd put this out there in the meantime.
And yes - I'm feeding it a mix-minus feed - but all that does is make sure the caller is not looping back to him/herself.

All the problems I've had with "ghosting" and feedback have been on the other studio's end. Even competent engineers can flip the wrong switch sometimes.
Other than that, I'll be no help at all.

you're right - it's not the unit.
Just got a call from Telos and the tech tells me that structurally that cannot be happening in the unit - so the feedback/ghosting is a result of the phone line.
He says they've been getting more and more calls about this due to the the telco's upgrading and changing their technology and switches.
So, to work around it, I put a bombfactory compressor on the bus coming from the Telos ONE in PT and duck in with my iso booth as the sidechain and it does the trick. A shame though - but it works.
